Role Summary
To provide specialist clinical pharmacy in a specific area of clinical and/or operational practice, to support the medicines-related care of patients, support the provision and development of medicines management services and to participate in training, education and research as required.
Responsibilities
- Take an active role in the pharmacy and medicines management team and provide support to less experienced staff providing a service to the Trust, acting as a clinical role model
- Seek advice from senior pharmacy colleagues in order to provide advice to clinicians on
unlicensed medicines use and areas of practice where the evidence base is poor - To work as part of a multidisciplinary team to provide safe, clinically effective and cost efficient use of medicines in patients, including participation in consultant ward rounds, and multidisciplinary team meetings. Provide highly specific advice to clinical team members on the management of complex medicines related issues, such as complex drug interactions, medicines use in pregnancy, unlicensed use of medicines etc
- Make appropriate referrals to the Pharmacist Team Manager (CHS & CMH, West & North Essex) and other members of the multidisciplinary team and within the pharmacy team
- Advise medical, nursing staff, and allied healthcare professionals on the actions, properties and the safe, cost effective use of drugs, and be able to critically appraise the literature and give accurate interpretation in order to improve use of medicines
- Utilise medicines information resources to further clinical pharmacy practice in community health services
- Provide advice, which is accurate, evaluated, impartial and up-to-date to ensure the safe, clinical effective and cost efficient use of medicines
