As the post holder you will contribute as a team member to the overall functioning of the service working directly or indirectly with a qualified nurse. As co-worker, the post holder will provide support to qualified key workers (RMN’s) in the assessment of client need and in the planning and evaluating of their care. A priority will be implementing planned care, working in partnership with clients and reporting back verbally and in writing to the key workers. The main objective of this is to enable our clients to participate fully in both home and community life. The post holder will participate, with clients in the general domestic routine of their home. This will include shopping, cooking and cleaning. The broad aim is to create as normal and homely environment as possible. The post holder will ensure that our services are provided in a way that is consistent with our values, supporting and promoting an organisational culture that values: compassion, accountability, respect, excellence, and innovation.
