Description

We are looking for a dedicated Locum Band 5 Community Nurse to provide high-quality nursing care to patients in their homes and community settings. This is a fantastic opportunity to work with a multidisciplinary team, supporting patients with a range of healthcare needs while promoting independence and well-being.

Key Responsibilities:

✔ Provide holistic nursing care to patients in community settings
✔ Conduct wound care, injections, catheter care, and medication administration
✔ Support chronic disease management, including diabetes and respiratory conditions
✔ Assist in palliative and end-of-life care for patients at home
✔ Work closely with GPs, therapists, and social care teams to coordinate patient care
✔ Educate and support patients and their families on health and self-care management
✔ Maintain accurate clinical records and ensure adherence to healthcare policies

Requirements:

🔹 NMC Registered Nurse (RN) – Band 5 Qualification
🔹 Previous experience in community nursing or district nursing (preferred)
🔹 Strong clinical assessment and wound care skills
🔹 Excellent communication and interpersonal skills
🔹 Ability to work independently and as part of a team
🔹 A valid UK driving license (preferred for community visits)
