Description
An exciting opportunity has arisen for a Care Coordinator to join our Team and support the Health Promotion Team and clinicians.
Your main duties will include, but are not limited to, the following:
- Organise and coordinate weekly rotas of care staff.
- Liaise with clients and their family on a regular basis.
- Ensure that the staff rotas are organised and sufficiently well planned, taking into account travel times and routes.
- Work with recruitment and Human Resources Teams to ensure sufficient current and future staffing levels.
- Plan and allocate care assignments to appropriate staff, with the upmost regard for the particular care needs of the client.
- Liaise, as necessary, with allied health professionals with the goal of delivering holistic, client-centred packages of care.
- Deal with queries from clients and families in a calm and professional manner at all times.
- Carry out on-call coordinating duties outside normal office hours.
- Regularly update client notes and care requirements as and when needed, as well as ensuring the client notes are prepared and in the home for all visits.
- Effective communication with care staff, ensuring they are meeting all of the client’s care requirements and are updating the office regularly.
- You may occasionally also be required to take on tasks outside the normal scope of this job description dependant on business requirements.
