The post holder will have a continuing responsibility for the care of patients in their care, whilst undertaking administrative duties associated with patient care. • To be active members of the Multi-Disciplinary Team and to attend weekly Complex Cases meeting and the referrals and allocations meeting. • To work with MDT in managing patients on the risk register, including organising and attending CTR meetings. • To participate and play leadership role in assessment and management of cases with moderate or severe LD and mental illness • To play a role in the interface with acute psychiatry in terms of mainstreaming services for patients with LD, which sometimes means joint working with adult services • To provide liaison with inpatient medical team for patients on Moore ward, including attending CPA and discharge planning meetings. • To provide in-service training and supervision to junior doctors and other
