We are looking for a qualified, NMC registered Learning Disability Nurse to support within our LDA division, skilled and experienced in supporting adults and young people within their own homes who present with learning disabilities and/or autism and may display behaviours that people may find challenging. The role of the LD Nurse will include: Effectively supporting the staff team with identifying and meeting complex physical and mental health needs of the client Provide clinical support to the client’s staff team, inclusive of reviewing clinical documentation (Careplans, MAR charts, risk assessments etc), to reflect changes in presentation. Working with partners to provide care that is centred on the individual, supporting them, their families and carers by signposting them to and helping them access hospitals and other mainstream services, in line with reasonable adjustments. Helping people to understand about their health and wellbeing.. Supporting the client’s staff team in understanding when they need to intervene with behaviour that challenges using Positive Behavioural Support. Using person centred assessments around complex physical and mental health needs and ensure that all interventions are focussed on improving quality of life. Liaising with other services and across all disciplines ranging from physical health services, health promotion, mental health, community services and social services
