Description
Your responsibilities
To work independently as part of a multi-disciplinary team to assess, plan and implement person-specific programmes of care/interventions within the Care Programme Approach, contributing to the continuous assessment of service users
Develop comprehensive discharge and contingency plans in partnership with service users, families and carers
Have an understanding of Housing legislation and the rights and responsibilities of service users with regards to Housing need
To be successful in this role, you'll need:
Professional Qualification (Degree or equivalent)
Professional Registration
Completion of ASYE
Experience within a similar role and setting
Knowledge and interest in MCA-MHA and specific clinical experience in mental health
Understand Risk Management, the purpose of Clinical Governance and the legal responsibilities of the profession
Previous experience in the implementation of safeguarding processes
