Description
• To participate as a full member of the multi-disciplinary team in the assessing, planning, implementation and evaluation of individual treatment and rehabilitation plans liaising with other relevant agencies as appropriate.
• To use an evidenced based patient centred approach to assess, plan, implement and evaluate treatment plans to support residents’ physical and psychological needs.
• Work with residents, and team members to identify goals and set treatment plans.
• Liaise with relatives and carers with empathy giving information, support and advice whenever and as often as required, in respect of treatment plans.
• Maintain concise records and provide reports as necessary for each resident’s case review.
• Identify risks and advise other team members on interventions to minimise risks.
• To undertake assessment of daily living skills to identify level of functional ability, risks and level of support needed.
• Participate in clinical meetings, reviews and other discussions where required.
• To ensure the cleanliness and maintenance of all Occupational Therapy equipment.
• Provide advice and support to staff by teaching relevant techniques to safely implement treatment plans.
• To be aware of and have regard for the Standards of Practice from the College of Occupational Therapy and other statutory bodies in relation to professional matters.
• To promote personal and social skills and leisure activities for all residents.
• To liaise with appropriate outside organisations for equipment provision e.g. wheelchair services.
• Undertake such duties as may be reasonably requested from time to time.
